Who is responsible for your information
Monira Dali, Registered Psychotherapist (CRPO #14315), operating as Dali Counselling, is the health information custodian responsible for the personal health information collected in the course of providing psychotherapy services. You can reach me at monira@dalicounselling.ca or +1 (226) 704-3040.
What this policy covers
This policy covers information collected through this website. Information collected during therapy is additionally governed by Ontario's Personal Health Information Protection Act, 2004 (PHIPA), the standards of the College of Registered Psychotherapists of Ontario, and the written consent you complete before your first session.
Information collected through this website
- Contact form. Your name, email address, phone number and whatever you choose to write in the message field. This is used solely to respond to you.
- Booking. Consultations are scheduled through Calendly, a third-party service. Information you enter there is governed by Calendly's own privacy policy, and Calendly may store it outside Canada.
- Website analytics. Aggregate, non-identifying statistics such as page views and referring sites, used only to understand which pages are useful. No advertising profiles are built and your information is never sold.
Please do not send clinical details by email or web form
Email and web forms are not secure channels and can be intercepted. Keep your first message general. There is no need to describe symptoms, diagnoses or history. We can cover all of that in the consult or in session.
How long information is kept
Website enquiries that do not lead to services are deleted once the conversation has concluded. Clinical records are retained for at least ten years after your last session, or ten years after your eighteenth birthday if you were a minor, as CRPO requires.
When information may be shared
Your information is confidential. There are narrow exceptions, which apply to every regulated health professional in Ontario, where disclosure is legally required:
- A risk of serious harm to you or to another identifiable person
- Reasonable suspicion that a child under 16 is or may be in need of protection
- Reasonable suspicion of abuse or neglect of a resident in a long-term care home
- Sexual abuse of a client by a regulated health professional
- A subpoena, warrant or court order, or an investigation by CRPO
I will discuss these with you at the outset so nothing comes as a surprise.
Your rights
You may request access to your records, ask that a correction be made, withdraw consent for the collection or use of your information, or ask a question about how it is handled. Write to monira@dalicounselling.ca and I will respond within 30 days. If you are not satisfied, you may contact the Information and Privacy Commissioner of Ontario or the College of Registered Psychotherapists of Ontario.
Security
This site is served over HTTPS. Sessions are held on a platform that supports encrypted video. Records are stored securely with access limited to me.
Changes to this policy
Any changes will be posted on this page with a revised date above. Material changes affecting current clients will also be communicated directly.
